Job Description Job Description Full-time, MSW opportunities in Virginia. Sign on bonus and relocation allowance available. Flexible scheduling options: Choose 4-10hr shifts 8am-6:30 pm weekdays, or 5-8hr shifts M-F 8am-4:30pm. The clinical social worker provides psychosocial assessments, interviews, and/or treatments and discharge planning to patients and families to assist them in coping with hospitalization, illness, diagnosis, and/or life situation. The social worker works collaboratively with other members of the health care team, community resources, regulatory agencies and third party payers in order to facilitate the transition of the patient through the organization while maintaining the highest quality of services possible. The social worker will adhere to the National Association of Social Workers Code of Ethics. The clinical social worker utilizes critical thinking skills, advanced problem solving, therapeutic interventions and advanced care planning in the day to day functions. Duties include, but are not limited to the following: Provide psychosocial assessments of patients and families. Formulating a comprehensive care transition plan coordinated with the interdisciplinary/ transdisciplinary team. Meets documentation standards for quality, timeliness and confidentiality of medical records in accordance with organizational and departmental defined policies and procedures. Remain current with social work and health care developments and seeks to increase further enhancement of job related knowledge. Facilitates and interdisciplinary approach to patient care. Provide therapeutic interventions aimed at adaptation throughout the continuum of care. Minimum Qualifications Education: Master’s Degree from an accredited school of Social Work. Experience: Minimum of two years social work experience in health care setting preferred, but willing to consider other relevant experience in the field of social work including internships during MSW work. Other Minimum Qualifications: Effective interpersonal, communication, and team working skills required. Must successfully complete orientation and competency validation for position. Demonstrates advanced knowledge of social work theory and practice: proven expertise as a clinical social worker; crisis intervention skills; critical thinking and creative problem solving. Satisfactory completion of orientation; positive interpersonal skills; oral and written communication skills; analytical skills; cultural competence; integrity; team player; courteous; ability/willingness to work as an integral member of a multi skilled team.
