Job Description:
Albany Medical Center seeks a dedicated Clinical Nurse III Post
Transplant Coordinator to join our leading-edge transplant team. In
this pivotal role, you will be responsible for the comprehensive
care of transplant patients, overseeing their journey from
pre-transplant preparation through to post-transplant recovery.
This position requires a deep understanding of transplant
protocols, exceptional interpersonal skills, and a commitment to
fostering a supportive environment for patients and their families.
Coordinate pre- and post-transplant care, ensuring all medical
protocols are followed. Educate patients and families on transplant
processes and post-surgery care. Monitor patient health and
response to treatment post-transplant, adjusting care plans as
necessary. Maintain accurate patient records and communicate
patient status to the transplant team. Collaborate with
multidisciplinary teams to facilitate comprehensive care planning.
Lead patient support groups and provide emotional support to
patients and families. Proficient in patient assessment and care
planning. Strong knowledge of transplant care and protocols.
Excellent communication and interpersonal skills. Ability to manage
multiple cases with attention to detail. Strong leadership skills
and teamwork orientation. Medical Insurance Dental Insurance Vision
Insurance Life Insurance Registered Nurse (RN) license in the state
of employment. Certification in Clinical Transplant Coordination is
preferred.
