social work Provide hospice and home health services with emphasis
on patient-centered care Participate in peer review audits and case
conferences to ensure quality outcomes Requirements: Active Vermont
State Board of Nursing RN license OASIS documentation experience
mandatory Home health or hospice nursing experience preferred
Proficiency in pain management, symptom control, and high-tech
nursing interventions Strong assessment, care planning, and case
management skills Ability to work independently while coordinating
