the Position TravelNurseSource is working with Cynet Health to find
a qualified Hospice RN in Bennington, Vermont, 05201! Job Title:
Hospice RN Profession: Nursing Specialty: Hospice Duration: 13
weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:00 PM,
with some weekends and on-call required Experience: Minimum 2 years
License: Registered Nurse (RN) Certifications: Current CPR
certification Must-Have: - Experience in hospice care - Strong
assessment and communication skills - Ability to manage caseloads
effectively Description: Utilizes the nursing process to assess
patient and caregiver needs in order to provide quality care to
assigned patients. Develops and communicates a realistic plan of
care for each patient, ensuring inclusion of patients and
caregivers in the care plan process. Serves as Case Manager for
assigned patients and actively manages caseloads and visit times in
accordance with agency productivity standards. Effectively
completes required documentation in accordance with agency policies
and procedures. Implements medical plans of care, nursing
interventions, and procedures to ensure safe and effective quality
of care. Maintains current knowledge of high-tech skills such as IV
use and maintenance, wound VAC, and complex wound care. Ensures
that infection control standards are maintained throughout the plan
of care. Works collaboratively with LPNs to ensure that the plan of
care is followed and delegates tasks appropriately. Evaluates
various aspects of the implemented plan of care to meet patient and
caregiver needs. Maintains accurate documentation of nursing care
to ensure effective integration of information for the healthcare
team. Follows agency protocols for various processes to maintain
efficiency and compliance. Educates patients and caregivers to
guarantee informed decisions regarding care options. Identifies
interdisciplinary needs of the patient and communicates these needs
to other disciplines in a timely manner. Completes necessary
agency, state, and federal forms accurately and timely during
visits. Carries out visits in a time-effective manner and
communicates visit-related information to the Scheduler as needed.
Assesses patients’ needs for home care services and develops a plan
of care in collaboration with the patient and caregiver. Transmits
patient incident reports to the designated team member in a timely
manner. Coordinates safe patient discharges and transfers, ensuring
all necessary paperwork is processed according to regulations.
Collaborates with physicians, providers, and team members to
support the recommended plan of care. Participates in monthly peer
review audits and accepts feedback professionally. Schedules and
facilitates case conferences with team members to ensure effective
communication. Advocates for patient care issues to ensure quality
and tailored care for each patient and family. Participates in
interdisciplinary team meetings to develop individualized plans of
care within specified time frames. Maintains up-to-date knowledge
of pain control and symptom management practices. Provides
comprehensive assessments to enhance quality care delivery to
assigned patients. Coordinates various aspects of patient care to
ensure safe and effective services for patients and families. 5
