Registered Nurse (RN) | Home Health Location: Rutland, VT Agency: Cynet Health Pay: $2,506 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Home Health RN Profession: Nursing Specialty: Home Health RN Duration: 13 weeks Shift: 8am to 4pm, M-F with some weekends and on-call required Hours per Shift: 37 Experience: Minimum of 2 years License: Current nursing license Certifications: OASIS experience required Must-Have: - OASIS documentation experience - Ability to assess patient and caregiver needs - Experience developing care plans - Knowledge of infection control standards - Ability to educate patients and caregivers Description: Utilizing the nursing process, assess patient and caregiver needs to provide quality care to assigned patients. Develop and communicate a realistic plan of care for each patient. Ensure that patients and caregivers are involved in the care plan process. Serve as Case Manager for assigned patients and manage case loads and visit times according to productivity standards. Complete OASIS documentation in accordance with policies and procedures and stay updated on documentation requirements. Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective quality of care. Maintain up-to-date knowledge of high-tech skills such as IV use, wound VAC, and complex wound care. Collaborate with LPNs to ensure the plan of care is followed and delegate tasks as necessary. Evaluate various aspects of the implemented plan of care, including identified problems and patient responses. Maintain accurate documentation of nursing care provided to facilitate effective integration of information for the healthcare team. Follow agency protocols and perform tasks related to visit completion and documentation. Conduct patient and caregiver education to ensure informed decision-making regarding medications and treatment options. Identify interdisciplinary needs of patients and document appropriately, communicating information to other disciplines in a timely manner. Complete various agency, state, and federal forms accurately and in a timely manner. Carry out visits efficiently and communicate scheduling needs to the appropriate personnel. Assess patients' needs for home care services and develop plans of care with cooperation from patients and caregivers. Transmit patient incident reports to the appropriate specialist in a timely manner. Coordinate safe patient discharges and transfers, ensuring all necessary paperwork and patient information are processed. Coordinate patient care with physicians, providers, team members, and vendors to support the recommended plan of care. Participate in monthly peer review audits and provide constructive feedback. Schedule, coordinate, and facilitate case conferences to maximize effective communication with team members. Advocate for patient care issues to ensure quality and type of care are sensitive to specific patient and family needs. Participate in interdisciplinary team meetings to develop individualized plans of care within designated timeframes. Stay updated on pain control, symptom management, and high-tech skills, ensuring infection control standards are maintained.
