Job Description:

Visiting Docs - Do you enjoy helping people? - -Do you want to be part of a caring, compassionate team? - -Do you have experience working with the elderly patient population?
We are looking for a motivated, hardworking, and experienced -Nurse Practitioner -or -Physican Assistant -candidate to join the growing Visiting Docs team.
The ideal candidate would have previous experience working with patients in their residences or in assisted living facilities. -
Responsibilities include but are not limited to: -
--- Providing general primary care to facility patients.
--- Diagnosing, treating and managing both acute and chronic conditions.
--- Prescribing and refilling medications as appropriate.
--- Actively communicating with patients, families, other specialists and facility personnel (case managers, social workers, med room, etc).
--- Ordering and interpreting diagnostics tests such as imaging and lab work.
--- Documenting patients' histories and treatments in EMR.
--- Seeing patients via telehealth, as needed.
Qualifications:
--- Active unrestricted New York state NP (must have board certification) or PA license.
--- Minimum three (3) years of experience preferred.
--- PECOS enrolled.
--- DEA certified.
--- BCLS.
--- Knowledge of eClinicalworks preferred.
Part time schedule with no evenings or weekends required. - Ideally schedule would be Monday-Friday 9:00 AM - 2:00 PM.
