Department/Unit:Access CenterWork Shift:Per Diem (United States
of America)The Referral Access Coordinator Senior is responsible
for the appropriateness of patient transfers between Albany Medical
Center Hospital and other health care facilities and settings. The
Coordinator screens the transfer based on established criteria and
matches the clinical setting to patient status. The transfer
process incorporates principles of Utilization Review and Case
Management.Qualifications and Ideal Characteristics: - - - - - -
-Must hold current NYS Registered Nurse license. Three years acute
care with diverse patient populations. Skilled/Literate in selected
computer programs. Knowledge of Utilization management/health care
reimbursement/medical terminology. Knowledge of EMTALA standards in
patient care situations. Demonstrates excellent communication,
collaboration and customer service skills. Demonstrates
problem-solving, conflict management, and negotiation

colleagues.Access Management and Triage Serves as the primary
contact for referring physicians who are requesting the services of
Albany Medical Center for continued patient care. Manages calls
received in the transfer center to provide a point of entry into
Albany Medical Center. Performs triage and prioritizes requests
received through the referral line. Initiates referrals and
inpatient transfers from physicians and medical professionals.
Provides assistance to emergent and non-emergent transfers,
consults, and other calls received. Maintains an awareness of
services and capabilities of Albany Medical Center facilities in
support of the outreach efforts of the transfer center. Maintains a
familiarity with internal systems available and necessary in the
performance of the job. Provides status reports to physicians,
administration, case managers and inpatient unit leadership of bed
assignments and outstanding admission requests. Serves as a point
of contact for Disaster Readiness in the region. This could include
surge capacity and bed availability for all Albany Medical Center
sites.Care Coordination Facilitates patient placement into and
within the Albany Medical Center Health System. Collects and
updates patient admission data within a variety of software used at
the various access points in the Albany Medical Center Healthcare
System. - Completes clinical screenings and registrations,
consulting procedure manuals as necessary. Requests and coordinates
patient transportation as necessary. Escalates issues, bed
assignments and problems as determined by department protocols.
Communicates STAT or emergent needs effectively up the chain of
command. Maintains records related to admission and transfer
effectively and in a timely manner. Provides detailed report to
oncoming shift to ensure continuity. Assists with orientation and
as a resource for new staff. Demonstrates self-regulation to
function calmly in emergency and high stress situations.Patient
Safety Supports a system that encourages the identification and
analysis of errors and near misses in the context of a safety
culture. As assigned, conducts departmental safety inspections to
assure compliance with hospital safety standards as evidenced by
documented inspection reports. Is aware of and compliant with all
hospital and regulatory agency safety goals and
objectives.Professional Skills:Maintains current knowledge of
utilization issues, changes and requirements. Supports and adjusts
to changes to the work environment. Contributes to change by
monitoring trends and suggesting improvement to process. Speaks
positively about change to others in and outside the unit. Seeks
professional education to improve job performance and maintains
competency through completion of required continuing
education.Reasoning Ability:Solves practical problems and deal with
a variety of situations where only limited standardization exists.
Identifies problems, collects data, establishes facts, and draws
valid conclusions. Interprets a variety of instructions in written,
oral, diagram or schedule form.Physical Demands:While performing
the duties of this job, the employee is regularly required to
stand, walk, and speak and hear. - The employee is occasionally

environment is usually moderate. This job requires as an essential
function that the majority of the time the employee be physically
