Seeking Full Time, Part Time, Per Diem Traveling Physical Therapist who is willing to travel within the Troy, NY area to provide evaluation and follow up visits in the home. Assisting patients in adapting to restricted or altered activities of daily living.COMPENSATION: Salary plus productivity bonus for full time employeesBENEFITS:Sign on Bonus of $2000Referral Bonus of $1200.00Generous PTO policyCDPHP health benefits401kLife insuranceJOB RESPONSIBILITIES:Provide physician directed physical therapy services by initially and periodically evaluating patient' s needs, establishing a goal directed plan of care and initiating planned care. The plan of care and re-evaluation occurs no less often than every 60 days.Provides quality care within guidelines of department policy and procedures as well as local, state and federal rules and regulations and Code of Ethics as established by the American Physical Therapy Association.Provides written documentation of assessment and care delivery in a professional manner according to policy and within designated time frame.QUALIFICATIONS:Graduate of a physical therapy school approved by the American Physical Therapy Assn., or the Council of Medical Education of the AMA, or the council on Medical Education of Hospitals for the American Medical Assn.Current Physical Therapy Licensure in NY State.Minimum of 2 years experience in a clinical setting.
