In TCB communities, we engage residents in creating programs and
build strategic partnerships with local stakeholders to maintain
stable housing and create pathways to opportunities in three key
practice areas: housing and financial stability, health and
wellness and social engagement.
Position Description:
Reporting to the Director of Community Life, the Community Life
Clinical Service Coordinator (CSC) develops and coordinates support
services for the for both senior and Non-Elderly Disabled (NEDS)
residents of Monument Square Apartments in Troy, NY. Monument
Square Apartments has 89 studio and one-bedroom units with 92
residents. Approximately, 20 percent of Monument Square residents
are between the ages of 20-44 years of age, 58 percent between the
ages of 45-64 years of age and 22 percent 65 years of age and
older. The service coordination will ensure resident and community
success through close work with property management, building and
sustaining relationships with local and regional partners, planning
programs and other on-site opportunities for residents, and working
one-on-one with residents to assist them in achieving their goals.
The CSC is an integral part of the housing and property team and
plays a critical role in the overall positive culture of the
property for the enjoyment of its residents and the respect of its
neighbors. The CSC will also be charged with meeting the metrics,
goals and grant requirements of the New York State Supportive
Housing Program (NYSSHP), which TCB was awarded and is designed to
provide supportive services to eligible residents of supported
housing in order to assist them in achieving as self-sufficient a
life as possible.
Community Life's Housing stabilization, health and wellness,
education, workforce development, financial literacy are areas of
focus that will be delivered through referrals by local partners,
on-site programing and individual coaching. The CSC will seek to
ensure that community engagement and community building activities
are integrate in the TCB development, creating a sense of community
and initiating a gateway to other support services. Specifically,
The CSC will provide and support partnerships to achieve vital
service provisions for low-income single adults whose mental,
development and/or physical disabilities make it hard to maintain
independence without onsite supportive services. All services will
assist our clients in achieving stable housing and more independent
lifestyles (e.g. programs addressing their ability to be hired as
well to remain employed, mental health stability, physical and
nutritional well-being).
The CSC will work with each of his/her older adults or non-elderly
disabled resident to create a success plan that will guide the
support, interventions, and referrals needed by the resident. The
success plans ensure a resident-centered approach to delivering
resident services. In addition, the CSC, in collaboration with the
Community Life HUB team, will track performance measures,
administer the annual survey (Community Life questionnaire),
develop a Community Success Plan (CSP), and evaluate effectiveness
of the partnerships to impact outcome measures.
The CSC will work with each of his/her resident to create an
individual success plan that will guide the support, interventions,
and referrals needed by the resident. The success plans ensure a
resident-centered approach to delivering resident services. In
addition, the CSC, in collaboration with the Community Life HUB
team, will track performance measures, administer the annual survey
(Community Life questionnaire), develop a Community Success Plan
(CSP), and evaluate effectiveness of the partnerships to impact
outcome measures.
Essential Functions:
Develop strategic partnerships with local service providers to
bring on-site programs or provide referral to off-site program and
services in the following CL outcome areas: housing and financial
stability, health and wellness and social engagement.
Track progress of NYSSHP grant goals as well as support grant
management team on NYSSHP compliance and reporting
Support TCB fund development team on supportive housing grant
applications and initiatives
Supervision of the social work interns
