  • Coordinate smooth patient transitions of care to home health services from Ellis HospitalResponsibilities:
    • Evaluates referrals for appropriateness for certified home health agency services
    • Describes services to patients/families to obtain agreement with services
    • Coordinates with the hospital team to ensure a smooth transition home with home health support
    • Contacts the patient's primary care physician for orders
    • Obtains signed Medicare Face to Face Encounter document from the hospital as needed
    • Data entry and scans information into the electronic medical record, and ensuring referral information is complete for the admitting clinician
    • Works closely with our Northeast Home Medical Equipment and Empire Home Infusion Pharmacy to coordination all home-based services (contingent upon patient choice and insurance authorization).What you will need:Required:
      • A current license to practice as a Registered Nurse (RN) in the State of New York
      • 2 years or more of recent experience as a RN ideally in home care, ED, ICU or oncologyPreferred:
        • 2 years or more of experience in a home care / community health setting
        • Bachelor's degree of nursing
        • Registered Nurses at Eddy VNRA must possess a passion for caring for patients and demonstrate a respectful, caring, professional demeanor
        • The RN must have excellent time management and organizational skills
