Role As Clinical Care Navigator you will work with members as part of an interdisciplinary team to ensure they are engaged and understand the aptihealth journey. You will ensure that the member experience is exceptional by guiding members early in the process, making sure to reduce any barriers that would prevent them from getting the right treatment at the time. You will use your clinical proficiency, along with the aptihealth platform to surround our members and assist in delivering the best possible outcomes. As a subject matter expert in all tools utilized by aptihealth, you will educate and support members as required. From point of referral to connection with a behavioral specialist, through treatment, you will coordinate and guide to assist members in feeling healthier, happier, and experiencing a better quality of life.
